How to be Referred

Your health care provider can refer you. You can find a link to a PDF referral form here.

You can self refer. For privacy reasons, this is best done by making an account on our secure patient portal here.
The Patient Portal
Once referred, you will be prompted to make a portal account. Using the portal you can:

Book appointments directly

Upload relevant forms and records

Receive forms to complete

Securely message your genetic counsellor
Pricing
The Appointments

Pricing for appointments is as follows (USD$):

Individual appointment (1 patient) $175
-
Individual follow up appointment (1 patient) $75
-
Group/couple initial appointment $112.50 per person (minimum 2) $225 and up

Group/couple follow up appointment $50 per person (minimum 2) $100 and up

Genetic test pricing varies according to the test.

Testing is usually done on a blood sample and will require that you attend a lab for blood draw.

All appointment and testing costs must be paid in advance.

Most people will require two total appointments.

Appointments are held using a video link via the secure portal.

Initial appointments are typically 50 minutes long
-
Follow up appointments are approximately 20-30 minutes long.

When your genetic assessment is complete, you/your physician will be provided with written documentation and copies of any genetic testing reports.
